財團法人國家衛生研究院（以下簡稱本院）實驗動物中心（以下簡稱本中心）因蒐集、處理及利用個人資料，依據《個人資料保護法》（以下簡稱個資法）第 8 條及第 9
條規定，向您告知以下事項，敬請詳細審閱。
1. 蒐集目的
本中心為辦理各項委託服務之需要，取得您的個人資料。
2. 蒐集個人資料之類別
蒐集之個人資料包括：姓名、機構、職稱、電話、電子郵件、地址及動物實驗計畫書相關資訊。
3. 個人資料利用之期間、地區、對象及方式
（1）期間： 依據前述蒐集目的之存續期間。
（2）地區： 本中心所在地。
（3）對象： 本中心及相關主管機關。

您可就您的個人資料向本中心行使以下權利：
（1）查詢或請求閱覽。
（2） 請求製給複製本。
（3） 請求補充或更正。
（4） 請求停止蒐集、處理及利用。
（5） 請求刪除。
惟若因行使上述權利而影響您的相關權益，本中心不負相關賠償責任。
5. 個人資料提供之影響
若您未提供正確之個人資料，本中心將無法為您提供相關業務服務。

The categories of personal data collected include name, organization, job title,
telephone
number, email address, mailing address, and information related to the animal use
protocol.
3. Period, Area, Recipients, and Methods of Use
• Period: The retention period necessary to
achieve the purpose of collection.
• Area: Within the jurisdiction where the
Center
is located.
• Recipients: The Center and relevant
competent
authorities.
• Methods of Use: Personal data may be
processed
and used in electronic or paper format or by other appropriate means.
4. Rights of the Data Subject Under Article 3 of the Personal Data Protection Act
Data subjects have the following rights regarding their personal data:
1. The right to inquire about or request access to personal data.
2. The right to request a copy of personal data.
3. The right to supplement or correct personal data.
4. The right to request the cessation of collection, processing, or use of personal
data.
5. The right to request the deletion of personal data.
However, if exercising the aforementioned rights affects the rights or services
provided,
the Center shall not be held liable for any resulting impact.
5. Impact of Not Giving Consent on the Rights and Interests of the Data Subject
Failure to provide personal data may affect the ability of the Center to offer
appropriate
services.
